We realize filling out paperwork for healthcare in today's world is no picnic. We have made every effort to minimize the paperwork required for our patients, while also getting the information we need to provide you with the highest quality service possible.

Appointments
Phoenix Physical Therapy and Sports Performance LLC. realizes that your time is valuable, and we will make every effort to respect your schedule while providing the highest quality care. For better service, please make an appointment during our office hours: Monday through Thursday from 8:00 AM to 5:00 PM and on Friday from 8:00 AM to 1:00 PM. Attendance
Patient attendance at all scheduled therapy sessions is essential to successful recovery. We ask that patients arrive at or before their scheduled time. If it is your first visit, check in at the front desk upon arrival.

 

Cancellations
Cancellations must be made 24 hours in advance of your scheduled visits so that we may use that time for other patients who are waiting to be seen. In cases of weather or illness we do not enforce this policy.

 

What to Wear
Depending on your injury, you should wear the following clothing to your appointments:

Fees
Applicable payments for services are expected at the end of each treatment unless prior arrangements have been made.

 

Insurance
At Phoenix we participate with most major and private insurance plans, including Workers’ Compensation and Medicare. As a courtesy, we will gladly file your claims for you.  Our staff will keep in contact with your referring physician, rehabilitation case manager or insurance company.
